神戸で開催された今年の泌尿器科学会では、初めて「ベックマン・コールター メモリアルレクチャー」として2演題の招請講演がありました。
まず、群馬大学の伊藤一人先生より、前立腺がんの新規マーカーとして注目される「proPSA」についての最新の研究成果のご紹介をいただきました。
さらに、オランダからDr. Monique J Roobolより、ヨーロッパでの研究成果に関して計算ツールの紹介なども含めた発表をいただきました。

ベックマン・コールターは、前立腺がんのマーカーである「PSA検査」のパイオニアとして、今後も研究活動をサポートしていきます。

『前立腺がんの新規マーカーの現状と展望』

現在、我が国で最も患者数の多い男性がんであり、また近い将来、罹患数でも胃がん、肺がん、大腸がんを抜き第1位になることが確実な前立腺がんは、早急に国家的な対策が必要であることは間違いない。前立腺がんの診断・治療システムの中でバイオマーカーである前立腺特異抗原(prostate specific antigen;PSA)は、これまで、がん罹患の予見因子、スクリーニング、がんの活動性の評価、適切な治療戦略決定、治療効果判定において極めて重要な医学的役割を担ってきた。PSA検査を用いた前立腺がんスクリーニング導入により、前立腺がん死亡率が大きく低下することが欧州の無作為化比較対照試験等で明らかになったが、がん死低下・転移がん進展抑制効果により優れ、費用対効果費が妥当な検診システムを確立することが重要である。そのためには、前立腺がん罹患の高危険群の同定、リスクに応じたPSA基準値・検診間隔の設定、精度の高い生検適応の絞り込みと生検手技、テーラーメイド治療戦略の確立など、我々が取り組むべき重要な課題は多い。将来の前立腺がんの検診・診断・治療システムは、より低侵襲な方向に進化していくべきであり、そのような見地からも、前立腺がんの新規バイオマーカーへの期待は大きい。
現在、前立腺がん診療の質の向上につながる研究成果が出ている有望な前立腺がん関連の新しいバイオマーカーは、尿中Prostate Cancer gene 3(PCA3) 、尿中TMPRESS2:ERG、そしてPSAの前駆体であるproPSAである。Beckman Coulter Memorial Lectureでは、同社が長年に渡り研究成果を蓄積してきた[-2]proPSA(p2PSA)を中心に、研究開発の歴史的経緯から最新の研究成果、現在進行中の研究計画を紹介し、また将来展望について考え、前立腺がん診療における、新規バイオマーカーとしての可能性を探りたい。
proPSAはPSAの前駆体として、正常な前立腺組織にも存在するが、がん組織ではPSAと同じhuman kallikurein family(hK)に属するhK2濃度の低下により活性型PSAへ変換が阻害される結果、proPSAが腺腔内に蓄積し、特にtruncated proPSAの最終型である[-2]proPSAがより多く蓄積され、微小血管浸潤によりproPSAの血中漏出が増加すると考えられることから、よりがん特異的なマーカーとして期待されている。
前立腺がんの実臨床で直面している課題を解決するために、世界で精力的に研究が行われているが、我が国発の注目されている重要な研究として、前立腺がん罹患予見因子としてのp2PSA関連インデックスの可能性を探索する症例対照研究を紹介する。さらに、効率的ながん診断と前立腺がんの活動性予測における優れたパフォーマンス、低リスク限局性前立腺がんの治療戦略として欠かせないPSA監視療法のより適切な症例選択と経過観察において臨床医の客観的・科学的な判断を支えるバイオマーカーとしての可能性について、最新の研究成果を紹介する。

『Screening For prostate cancer:Data from ERSPC Rotterdam, secondary analysis and risk stratification.』

The first report of the European Randomized study of Screening for Prostate Cancer (ERSPC) appeared in 2009 showing PCa specific mortality reduction of 20% in favor of men randomized to the screening arm (S-arm). Two years later a 21% PCa mortality reduction with an increased level of significance after a median of 11 years of follow-up was found. After unblinding of the ERSPC trial the reporting of end point related results of the 8 individual centers was allowed. ERSPC Rotterdam identified a total of 88,283 men aged 54-74 years in the population registry and invited these men to participate. A total of 42,376 men responded (48%), and were randomized to the S-arm (21,210 men) or C-arm (21,166 men). After a median follow-up of 12.8 years systematic PSA based screening reduced PCa specific mortality by 32% in the pre-defined ERSPC core age range 55-69 yrs (Intention to screen analysis (ITS)). The roughly 2-fold higher incidence in the S-arm due to active screening and the positive predictive value of the prostate biopsy of 24% when applying a PSA cut-off of >= 3.0 ng/ml as indication for biopsy underlines the importance of tools to better identify those men who would benefit from screening.
Although the conventional ITS analysis provides the best estimation of the PCa-specific mortality reduction on a population-based level, the potential effect of screening for an individual choosing to be screened needs to be corrected for nonattendance in the S-arm and contamination (eg, PSA testing/prostate biopsy) in the C-arm. An analysis correcting for nonattendance and contamination using a statistical method that preserves the benefits obtained by randomization resulted in a 51% PCa mortality reduction in favor of screening. This means that PCa screening as conducted in the Rotterdam section of the ERSPC can reduce the risk of dying from PCa up to 51% for an individual man choosing to be screened repeatedly compared with a man who was not screened.
The diluting effect of contamination and nonattendance is not exclusively limited to the effect of screening on PCa mortality. The downsides of screening: unnecessary testing and overdiagnosis (with subsequent overtreatment), is watered down as well. It should therefore be recognized that although screening as conducted in ERSPC Rotterdam (PSA cut-off = 3.0 ng/ml for biopsy, 4-year interval) has a major effect on the PCa mortality of an individual men, PSA-based screening is still far from ideal. If screening is asked for, already available tools, such as risk calculators, should be used to reduce overtesting and overdiagnosis. One of these very promising tools is the Rotterdam Prostate Cancer Riskcalculator ( www.prostatecancer-riskcalculator.com) which has been shown to be able to reduce unnecessary biopsies with more than 30% and is capable to selectively identify potentially aggressive Pca. This risk calculator is also available as a mobile application (app) providing doctors and patients with an increasingly powerful tool which is easy to use and always accessible.
